ADDING YOUR NEWBORN BABY TO YOUR INSURANCE
Your newborn must be added to your insurance within 30 days of birth. If not added by the employer imposed deadline your child will have no insurance coverage until the next employer insurance open enrollment period.

We accept most commercial insurances including most Blue Cross Blue Shield Plans, United Healthcare, Viva, Cigna and Aetna. However, prior to arrival, please contact your insurance company to assure that we are current providers with your specific plan.
MEDICAID AND TRICARE
We are not Medicaid or Tricare providers.
APPLYING FOR ALLKIDS INSURANCE
While applying for ALLKIDS your account will be considered a private pay account. If ALLKIDS approval is received we will subsequently file all claims for services received -- back to the effective date on the ALLKIDS insurance card.
CHILDREN WITH TWO INSURANCES
If your child has two insurances you must contact both insurances to coordinate the benefits prior to your first visit with double coverage. Only the insurance companies can designate which of the two is Primary and which is Secondary.
HAVE INSURANCE BUT NO INSURANCE CARD
In order to file a claim for services received our business office must have the following information:
- insurance company name
- contract, ID, policy, or member number
- group number
- policy holder’s name and date of birth
- effective date of the insurance
This information may be obtained either from the employer or directly from the insurance company.
LIMITED ACCESS TO PATIENT INFORMATION/CHART/ACCOUNT INFORMATION
A CONTACT SHEET must be completed for each patient. Only those listed on the Contact Sheet will be allowed access to patient information, including but not limited to: billing office information, lab results, appointment information and chart notes unless otherwise allowed or required by law.
All patients age 14 must individually complete a CONTACT SHEET which must be signed by the patient. Otherwise no one including the patient’s parent will have access to patient information.
Health Insurance Portability and Accountability Act (HIPPA)
Birmingham Pediatric Associates complies with the Health Insurance Portability and Accountability Act of 1996 (HIPPA). The HIPPA law addresses the privacy and security of confidential health information. Upon your first visit to Birmingham Pediatrics, you will receive our Notice of Privacy and Practice as required by federal law.
With Birmingham Pediatric Associates adhering to HIPPA, your child's health information will remain confidential. In compliance with HIPPA, you must provide a written permission for us to release any of your child's medical information. To protect your child's privacy, we may not be able to fax some requested information including school excuses or immunization records without your written permission.
