In order to view or print these forms, you will need Adobe Acrobat Reader installed. Click here to download it.
FOR EXPECTING PARENTS
Your newborn must be added to your insurance within 30 days of birth. If not added by the employer imposed deadline your child will have no insurance coverage until the next employer insurance open enrollment period.
We would like to invite you to attend our open house for expectant parents. It is held monthly and is usually the first Monday of the month at 6:00 p.m. If it falls on a holiday, it will be held on the second Monday. You will tour the office, meet the physicians and learn what to expect when your baby is born. There will be a 30-minute presentation given by one of our Pediatricians. There will also be an opportunity to ask questions.
